What is botulinum toxin?

 

Botulinum toxin is produced by gram-positive anaerobic bacteria Clostridium Botulinum. Botox is a trade name used by Allergen for Botulinum toxin and basically it is a purified form of botulinum toxin which has both aesthetic and therapeutic uses. The trade name Botox has become so popular that people recognize trade name better than its actual name; just like we use the trade name Xerox for photocopy. Many of us get afraid to hear the word toxin but botulinum has the real role to play in dental treatments. It is the toxin that helps to age gracefully.

 

Are there any variants of Botulinum Toxin?

 

Yes, there are seven type of botulinum toxins which are available to us. It is from type A, B, C, D, E, F & G. The one which is used most commonly is type A; however if any individual gets resistant to type A then we can use type B botulinum toxin. The type B botulinum Toxin is known as Myoblock.

Botulinum Toxin is available to us from different pharmaceutical companies.  There are several other trade names of botulinum Toxin like Desport, Neuronox and there are many more apart from Botox.

 

How Botulinum Toxin works?

 

Botox has its action on the muscle; it acts on SNAP 25 and doesn't let the muscle contract. It takes around 1.5-2 hours to bind to the nerve and it can take anywhere between 2 to 10 days to show full clinical effect. The effect of botox last from 3 months to 6 months varying individual to individual.

 

Some common Myths & Facts about Botulinum Toxin

 

Myth: It is new and not tested.

 

Fact: Botulinum toxin has a history of more than 20 years. And there are thousands of research papers available on the same.

 

Myth: It will give us botulism or else I will loose sensations and will have a plastic face.


Fact: None of it is correct. The lethal dose of botox is 2500 units to 3000 units which is 25 to 30 vials which is a lot of money to kill someone. A 100 unit vial of botulinum Toxin costs somewhere between Rs.18,000- Rs.20,000 INR

 

What do I have to know for getting good results from botulinum toxin?

For getting good results from botox injection one needs to understand the anatomy of the face or the areas where they want to inject botox.

Financial Management: "Professional Tax" For Dentists

I have come across many incidences where professionals had to pay additional interest to the government as they failed to pay their professional tax and this happens because most of them were unaware of Professional Tax. Some also have misconception that taxes are to be paid only when their income comes in the tax bracket & they approach CA only when their income becomes taxable under Income tax. This blog is an attempt to expound the concept of professional tax and help you save on penalties.  

 

What is Professional Tax Or PT?

PT is a tax charged on the income earned by way of profession or employment. This is additional to income tax and has to be paid to State Government. Professionals like doctors, lawyers, chartered accountants, artists etc comes under the purview of this tax. This is a slab-based tax and the rates vary state to state however the maximum amount that can be charged as PT by any state is Rs. 2500 annually. 

 

Registration for Professional Tax 

It is advisable to register your practice for PT immediately after commencement of dental practice. Before registering yourself for PT, you need to understand two main things one is PTRC and the other is PTEC. 

Professional Tax Registration Certificate (PTRC) is to be obtained by the professional in the capacity of employer. For instance, if you have 10 employees working with you in your business having monthly salary more than 7500, then you need to deduct PT on their salaries every month as per the rate given (Rs.200 per month & Rs.300 for February month).

Professional Tax Enrollment Certificate (PTEC) is to be obtained by the professional for its own existence and engaging in profession (business) within the state of Maharashtra.

PTR will have to be paid according to the salary paid to employees whereas PTE is fixed at Rs.2500 annually.

 

How to register? 

Step 1: Log on to www.mahavat.gov.in and open ‘e-PT Registration’ in e-services and fill in details like Name of the applicant, PAN, TAN, Status of Applicant and Location. Select Act as ‘PTRC/PTEC Registration’ (any one which you have to register for), the e-application for registration in ‘Form I’ will open for PTRC and ‘Form II’ for PTEC. Fill in the complete details and ‘Submit’.

 

Step 2: Once the application is submitted; an acknowledgement is generated containing date and time for attending before registration authority along with designation and address of registration authority. This acknowledgement has to be printed and signed by the applicant and produced before the PT registration authority at the time of verification on given date and time along with relevant documents. Once it is approved by the authority, TIN (Registration Certificate Number under the profession Tax Act) is generated. 

 

Step 3: After obtaining Registration Certificate the certificate holders are now needed to enroll themselves for the e-services on the same website, this will enable them to file e-return and e-payment of professional tax. 

 

The due date for paying PT is before 30th June of every financial year.

 

Consequences of late payment or non-payment of PT:

For late payment of PT, interest @1.25%-2% pm is levied and the state may also impose a penalty of 10% of the total amount due.

If you ask me, I would say the earlier considerations are basic which will be there before joining any University abroad. But, the real things will come once you join your MS. I stayed in Melbourne for 3 years and my journey was amazing. It was full of ups and downs. It taught me about being independent, being fearless, to forget and to forgive and many things about love and life but the most important thing it taught me about is, with discomfort comes growth. With every challenging situation, comes a great lesson. It helps you to evolve as a person. To believe in yourself and start everything from scratch in a new country, on your own, is difficult. But the moment you get succeed, that powerful feeling has no comparison in the world. People will always tell you how it has changed their life for good but no one talks about the pain they have gone through. 

 

No matter how supportive your parents are, leaving far away from them and the fact that they are getting fragile and old will always make you feel guilty. At times, you might feel lonely and depressed, no matter how many friends you have and how often you socialize. There are times when no one is around and you need to deal with it on your own. So if you are thinking of pursuing Post graduation or MS from abroad after BDS; think about all the factors and make a wise decision.

These issues are very sensitive, due to some malpractices or negligence of doctors and also because some hospitals are trying to get more money by recommending unnecessary medical tests and treatments, people started looking at doctor’s community with distrust. It is found that there has been a rise in the patient’s awareness and the judiciary authorities are more consumer-friendly now and have been asking doctors or hospitals to compensate their mistakes.

According to a 2013 survey by Dr.Ashish Jha of Harvard School of Public Health, India recorded 5.2 million medico-legal cases in a year, for an incorrect prescription, wrong timing, and errors in surgery, incorrect drug, and dosage. Therefore, to protect doctors from the above financial risk, a special financial cover known as Professional Indemnity Insurance (PII) is introduced.

 

What is Professional Indemnity Insurance?

Doctors are the professionals who face the risk of financial liability while practicing their profession. The amount they can be sued can be very high. Therefore they need a special financial cover to protect themselves financially when they are sued for unprofessional conduct, negligence or mistakes during treatments.

In short, Professional Indemnity Insurance (PII) is an insurance policy specially meant for professionals like doctors, lawyers, Charted Accountants etc to cover liability falling on them as a result of errors, negligence, mistakes committed by them while rendering their professional service.

 

What does PII policy cover?

The policy covers all sums which the insured Doctor becomes legally liable to pay as damages to the third party in respect of any error or negligence committed while rendering professional service. Professional Indemnity Policy covers against following:

1. Professional Negligence

2. Unintentional breach of copyright or confidentiality

3. Loss of documents or data

4. Claim investigation costs

5. Offense & insult

6. Loss of goods or money


What PII does not cover?

1. Intentional damage

2. Claims and circumstances known before issuance of the policy

3. Act of Terrorism or war

4. Contractual liability

5. Insolvency of the insured

 

 

Which insurance companies provide PII for Doctors in India?

Insurance companies like United India Insurance, The New India Assurance Co. Pvt. Ltd., Reliance General Insurance, ICICI Lombard GIC Ltd, offer PII policy coverage for Doctors in India.

How to select the sum assured?

In professional Indemnity Policy, the sum insured is referred to as Limit of Indemnity. This limit is fixed per accident and per policy period which is called Any One Accident (AOA) limit and Any One Year (AOY) limit respectively.

The AOA limit, which is the maximum amount payable by the doctor for each accident, should be fixed depending upon the nature of treatment and the worst possible damage that could happen to the patient.

 

How to claim?

In case you feel that any event will likely to give rise to a liability claim, you should inform your insurance company immediately.

In case you receive any legal notice or summons, it should be sent to the insurance company. The company has the authority to arrange defense on your behalf.

The maximum amount payable will also include defense cost which will be included in AOA limit selected. The AOY limit will get reduced by the amount of claim paid for any one accident. Any number of such claims should not exceed the AOY limit.


How much does it cost?

Normally the premium for PII is 0.30% to 1 % of the amount insured. So if you wish to get a professional indemnity insurance for sum insured of 5 Lakh; then your premium may range from Rs.450 to 5000 The premiums may vary from company to company as well as depending on the doctor’s specialty, seniority, practice, equipment used etc. majorly. It will be a year policy, every year the doctor has to enter into a fresh agreement.

 

What if you don’t have a PII cover?

Indian patients treat doctors as next to God, but now the situation is changing. The fact is that Doctors are also humans who can commit mistakes during treatment. Hence it completely makes sense to have professional indemnity insurance to secure your dental/ medical practice.
